Leaders at Blessing Health System can fill out the form below to receive "We Are Blessing" t-shirts handed out during leader rounding, Nurses Week, or Hospital Week picnics for employees who may not have had their desired size available. Select the sizes you need, and enter the quantities of each shirt size needed.
Communication will be sent out to leaders once additional t-shirts have arrived.
