Get your family involved for the 2018 Illini Community Hospital Ugly Sweater 5K! The run takes place Saturday, November 24, with registration taking place at Illini Fitness, located at 128 W. Washington in Pittsfield, Illinois.

Registration begins at 9 a.m., and the run itself begins at 10 a.m. Race fees are $20 for Adults. Kids 12 & under are $5 with shirt, free to kids who do not order a shirt. (NOTE: Guaranteed t-shirts are now not available to those registering after Noon on Nov. 13. Those who have registered before Noon on Nov. 13 will receive t-shirts)

Please list the sizes, and how many of each are needed, for Ugly Sweater 5K shirts below. Sizes for adult shirts range from Small-2XL, and kids sizes range from S(6-8), M (10-12) and L (14-16).

Photo Release: I consent to Blessing Health System (BHS), its employees or designees to photograph me during the Illini Fitness Ugly Sweater 5K and use for publication including, but not limited to BHS’s website, facebook page and other publications. I consent and waive all rights and claims for payment or royalties in connection with the use of the photographs. I can revoke the consent at any time, however, the hospital is unable to revoke information that has already been disclosed.

I, on behalf of myself, my personal representatives, heirs, executors, administrators, agents, and assigns, HEREBY RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE ILLINI FITNESS, its governing board, directors, officers, employees, agents, and volunteers, (hereinafter referred to as "Releasees") for any and all liability, including any and all claims, demands, causes of action (known or unknown), suits, or judgments of any and every kind (including attorneys' fees), arising from any injury, property damage or death that I may suffer as a result of my participation in the Activity, REGARDLESS OF WHETHER THE INJURY, DAMAGE OR DEATH IS CAUSED BY THE RELEASEES, UNLESS THE INJURY DAMAGE OR DEATH IS CAUSED BY THE RELEASEES’ NEGLIGENCE OR INTENTIONAL ACTS, AND REGARDLESS
OF WHETHER THE INJURY DAMAGE OR DEATH OCCURS WHILE IN, ON, UPON, OR IN TRANSIT TO OR FROM THE PREMISES WHERE THE ACTIVITY, OR ANY ADJUNCT TO THE ACTIVITY, OCCURS OR IS BEING CONDUCTED. I further agree that the Releasees are not in any way responsible for any injury or damage that I sustain as a result of my own negligent acts.
