Our Post-Discharge Clinic is open to provide hospital follow-up visits for patients who do not currently have a primary care provider (PCP). These patients often require close follow-up within a short window after discharge, and this clinic ensures they don’t fall through the cracks—helping improve continuity of care and reduce hospital readmissions.
