Illini's Next to Home Recovery program is designed to bridge the gap between the hospital and home. This extended care program provides the additional time to rebuild strength and recover following an acute illness or surgery. Today's reduced hospital stays often mean further recovery and rehabilitation must continue after you are discharged from the hospital. Next to Home Recovery allows you to recover close to home in a safe, comfortable atmosphere. 

With Our Swing Bed Program, you can expect to receive:

  • 24/7 nursing care
  • Therapy, laboratory, respiratory, radiology and wound care services as applicable
  • Individualized care plan designed with input from both you and your care providers

Your goals are our goals

We want to assist you in becoming as independent as possible for the transition to home, assisted living or long-term care. Next to Home Recovery patients are encouraged to dress in their normal clothing, eat meals out of bed and participate in daily therapy rehabilitation and activities. Through the collaboration of our therapists, nurses and coordinators, we can help you reach your goals.

How to qualify

A patient must have a skilled need such as physical therapy, occupational therapy, daily wound care or IV antibiotics. The patient must have skilled benefits through their private insurance or their Medicare replacement policy. Your physician or primary care provider will help you determine if you qualify, under federal guidelines, for the program. Our Next to Home Recovery coordinator and care team will review your referral to ensure that our program is the appropriate level of care for you in order to best meet your needs.
