A referral for your persistent pain must come from your primary care provider. During your initial visit, a plan of care is established, and personal goals are developed. Follow-up visits focus on response to treatment, pain status, medication management, and function.
Patients are referred to physical and occupational therapy or behavioral services as needed. Psychological and peer-to-peer support facilitated by our social worker and nurses are regularly offered and readily available.
