One way all hospitals are measured is by 30 Day Readmissions. A 30 Day Readmission occurs when one of our patients has a hospital stay and then has a condition that requires them be readmitted to the hospital within 30 days of when they left the hospital the first time.
Sometimes, there is not anything that can be done to keep the patient from being readmitted. However, there are other times that something could have been done better to help make sure the patient wouldn’t have to come back to the hospital so quickly. Blessing does many things to keep patients from being readmitted. For example:
- Follow-up to make sure the patient understands what they must do when they are discharged from the hospital
- Follow-up to make sure the patient gets the prescriptions for all new medicines filled when they leave the hospital.
- Follow-up to make sure the patient makes an appointment and sees their Primary care Physician after a hospital stay.
- Provide Home Health Care to the patient to make sure the patient continues to get better.
- Identify patients with chronic diseases who may require extra help in managing their disease and provide help.
