At this visit, you and the team’s provider, will discuss your medical history and determine what preventive services you may need to maintain personal wellness.

Our highly skilled providers encourage you to make an appointment and take advantage of your Annual Wellness Visit, a benefit of Medicare beneficiaries. Our provider team collaborates with Blessing's Annual Wellness Visit team to perform the important functions of the visit, including the development of a personalized care plan to assist with improving the quality of your health.

  • What will happen at the Annual Wellness Visit? The visit is for talking with your health care team about your medical history, your risk for certain diseases, the current state of your health and your plan for staying well. We will ask you health-risk screening questions during your visit, which includes questions about your living environment, diet, exercise habits and daily activities. We will measure your height, weight and blood pressure. We will perform screenings to detect depression and risk for falling. Based on the information gathered during this appointment, we may discuss additional Medicare screening and preventive services available to you, as well as provide you with information to address other health concerns.
  • How is the Annual Wellness Visit different from other visits? This is not the same as a yearly physical exam. You will not get screenings, such as blood tests, during this visit unless your provider feels they are necessary. We would need to schedule another appointment if you are not feeling well or are concerned about a medical problem.
  • Do I have to pay for the visit? Based on your Medicare eligibility, Medicare will pay for the Annual Wellness Visit, and you will not have an out-of-pocket expense unless there are additional tests ordered by your Annual Wellness Visit team. You might have a co-payment for some of the screening services and follow-up visits, as appropriate, for additional services or screenings. If you receive additional tests or services during the same visit that are not covered under these preventive benefits, you may have a co-pay, and the Part B deductible may apply.
  • Things to bring to your Annual Wellness Visit:
    • A list of all medications you take (with dosages), including vitamins, herbal supplements and over-the-counter medications
    • A list of all health care providers whom you currently see and any medical supply stores that you use
    • Your immunization records, if available
    • A living will or any advanced directives you may have
