WAIVER, RELEASE OF LIABILITY AND ASSUMPTION OF RISK
In exchange for my being allowed to voluntarily participate in (“Race/Program”), which may be conducted in person or virtually, I agree to be bound by the terms and conditions listed below:
I acknowledge that I have either had a physical examination and have been given my physician’s approval to participate in the Race/Event , or that I have decided to participate in these activities without the approval of my physician and do so voluntarily and assume all responsibilities for my decision to participate in this Race/Event. I understand I may withdraw from the Race/Event at any time or stop an activity at any time of my choosing and that when done as a virtual event, I may have the ability to participate whenever, wherever, and however I choose. .
ASSUMPTION OF RISK: I understand that my voluntary participation in the activities of the Race/Event, may include running on a road, trail, treadmill or walking, which is of my personal choice, all of which I acknowledge can potentially be very challenging and lead to injury, disability, illness, discomfort and in some cases even death. I affirm that I am physically and psychologically capable of participating in this Race/Event and that I am not aware of any condition, impairment, disease, illness or other limitation that would prevent me from safely participating in this Race/Event. I hereby assume all responsibility for any possible risk of injury and even death, which could occur as a result of my voluntary participation, whether walking or running.
WAIVER AND RELEASE: I, individually and on behalf of my heirs, beneficiaries, relatives, personal representatives and assigns, release, hold harmless of any liability and forever discharge, BLESSING HOSPITAL (which include BLESSING CORPORATE SERVICES, BLESSING HEALTH SYSTEM and all related entities and their employees, representatives, directors and officers, and agents (hereinafter referred to as “BLESSING”) from any and all claims of liability, injury, loss, expense, or damage in any way connected with my participation in this Race/Event. I intend for this waiver and release to also apply to my relatives, personal representatives, heirs, beneficiaries, next of kin, and assigns who might pursue any legal action or claim for such liability, loss, injury, or damage arising from my participation in this Race/Event, as well as agree to be fully responsible for anyone I registered for under the age of 18.
MEDICAL TREATMENT: I agree that BLESSING is not responsible for any medical or legal fees incurred as a result of my participation in this Race/Event are my responsibility. I understand I will be responsible for any and all costs associated with medical treatment of any injury, condition or damage that may arise from my participation in this Race/Event.
PHOTOS:I agree that any photos, whether in still form or video recording, that I may provide to Blessing may be used by Blessing on their Facebook page or any other social media sites. I hereby provide permanent permission to use my photograph, voice, recording and likeness taken in relation with this Race/Event, in any form. I waive any right of inspection or approval prior to Blessings use and understand such photographs and recordings shall become the sole property of Blessing.
I have read, or had read to me, this Waiver/Release of Liability form. I fully understand and agree to the contents and am signing this waiver voluntarily. I understand this to be a full, permanent, complete and unconditional release of all liability for myself as well as for any participant under 18.
